For residents of „Claudiushof“ in Reinfeld, we are happy to take over the nursing care routines. Starting in October 2024, PNT Pflegedienste will also be implementing the services for assisted living. Support within the facility is available at any time if needed.
The process begins with a comprehensive consultation that takes your needs into account. Together with you, we will explore all possibilities to ensure your care at home. How do I get outpatient care services?
Even without a care level, you can receive medical care services – your family doctor will prescribe them for you. To be able to support you in your household or with everyday activities such as personal hygiene, dressing, or eating, you will need a care level. However, before the MDK comes to assess you, we will have an in-depth consultation with you and discuss all the options for how we can help you as quickly and comprehensively as possible. Of course, we will also accompany you through the application and assessment phases.
